Discharge and aftercare planning are important components of our mental health treatment and recovery. An effective discharge plan starts on the day of admission. The providers are not the only ones responsible for our treatment, recovery and discharge – working with our treatment team, we can express our needs and wants and help design a comprehensive plan that works best for us. We will discuss what information to provide and what questions to ask the treatment team to plan for our discharge and continue our recovery after leaving the facility.
