When one is hospitalized in a psychiatric facility, it’s common to feel confused. We might not understand our treatment plan, discharge plan or aftercare plan. We might not know who is on our treatment team or your discharge team, who to go to for different aspects of our care. We might not know how to prepare for discharge, what plans need to be made in preparation for release. Understanding these components can help us in our recovery.

Discharge and aftercare are important components of our mental health recovery. Preparing for discharge and aftercare begins the first day of hospitalization, and a successful treatment team includes the individual receiving care who is an essential part of the team. We’ll talk about how to understand the process of discharge and aftercare and how to prepare for release with specific questions you need to keep in mind.
