We believe collecting information from you about your race, ethnicity, age, sex and sexual orientation will help us to understand how people from diverse backgrounds access services. We would greatly appreciate it if you share this information with us, but we understand if you choose not to. 

We have short ASL videos that explain the survey. You can share feedback in two ways: call us to set up a Zoom meeting in ASL, or record yourself signing your feedback and upload the video to our secure portal. We are glad to hear from you.  If you want to upload a video please use our secure portal.
