SB 214 (Dodd)
Requires the State Department of Health Care Services (DHCS) to implement and administer the California Community Transitions program (CCT), as authorized under federal law and pursuant to the terms of the Money Follows the Person Rebalancing Demonstration. This bill requires DHCS to seek federal matching funds and to fully fund the program if matching funds are unavailable. Established in 2007, CCT made funding available to help low-income people with disabilities transition from nursing homes to their own home or another community setting. Federal funding was set to expire at the end of 2018, but Congress recently approved a brief extension. Future federal funding is uncertain. There will also be a companion budget proposal submitted to the budget committees.
